TENNESSEE
4-H JUNIOR HIGH
ACHIEVEMENT RECORD GUIDELINES |

A table of contents section is not necessary. However,
it is helpful to the judge if the member will include a division
page for each section of the book. The use of a tab on each division
page is encouraged.
The contents of all junior high 4-H achievement record books should
be as follows:

Tennessee 4-H Report Form. The Tennessee
4-H Achievement Record Report Form (F305) should be the first
form in the book. Include forms for the past year plus the
current year. ONLY THE REPORT FORMS FOR THE PROJECT THE MEMBER
IS ENTERING IN COMPETITION SHOULD BE INCLUDED. Report forms
for other projects should be kept by the member, but should
not be in the achievement record book which is submitted for
competition. DO NOT ADD PAGES TO THE REPORT FORM.

Project Records - Main Project. Project
plans (F652) should be included in the front of each year's
records. Include project manuals for the past year plus the
current year's manual along with any supporting material requested
in the manuals. Use the project plan as the divider between
each year's activities. The entire project manual should be
included. F540 - Tennessee 4-H Project Record Worksheet or
other special project records may be included in order to
show that the member has completed the requirements stated
in the project manuals. |

4-H Story. The 4-H story is limited
to one page. The story should be written in parts as follows:
| Part 1 - Introduction
Part 2 - Main project
Part 3 - Other projects |
Part 4 - Leadership
Part 5 - Citizenship
Part 6 - Plans for future & career goals |

Project Pictures. Member may include
one page of pictures illustrating the highlights of the main
project. |

Other Project Records. Leadership
and citizenship project records and/or manuals may be included
for the past year and the current year if the member was enrolled
in those projects. Records for projects other than the main
project, leadership and citizenship should not be in the achievement
record book. (When entering leadership,
other project records would include citizenship and one other
project. When entering citizenship,
other project records would include leadership and one other
project.) |
Record Cover
The member may use the standard record book cover available from the
National 4-H Supply Service or use any other kind of cover, folder,
etc. as long as the contents are securely fastened and the book will
withstand the wear and tear of the U.S. mail service. Three ring binders
are not recommended because they are more bulky when shipping and
are more likely to come open and lose some of the contents.
Format
The Tennessee 4-H Report Form and the story may be completed
with a typewriter, word processor or may be hand written. The form
is available from the Tennessee 4-H Web site at www.utextension.utk.edu/4H/forms&materials/
and can be completed online. If a typewriter is used, the type should
be no smaller than 12 characters per inch (elite type). If a word
processor is used, use one standard typeface such as Times in a
12-point or larger size. Members may copy the Tennessee 4-H Report
Form on a disk, complete the form and print a copy for the record
book. Keep the headings and spaces as close to the original as possible.
Please bold and/or underline the headings and instructions. It is
not necessary to copy the lines if record is to be completed on
a word processor. UNDER NO CIRCUMSTANCES MAY PAGES BE ADDED
OR A DIFFERENT FORMAT FOR THE FORMS BE USED.
