Senior 4-H citizenship and leadership
record books are due in the state 4-H office
no later than February 2, 2004. |

A county may enter up to three
books in each Level I Leadership, Level II Leadership, Level
I Citizenship and Level II Citizenship for a total of twelve
books. |

Junior high books are to be submitted to the district
on the date announced by each district. Junior high books will
not come to the state. The contents of a junior high record
would be the same as senior record books except for the number
of years records may be included. Junior high books would only
include records for the past year (2003) and for January 2004. |

Senior members entering leadership or citizenship should include
a Tennessee 4-H Report Form 305 for each year
they have been enrolled in leadership or citizenship up to a
maximum of three years (2001, 2002, 2003). An additional
form should be included for activities for 2004. Size
and scope section in the forms may be omitted since all size
and scope for leadership should be reflected in Section II of
the form and size and scope for citizenship would be reported
in Section III. |

In addition to the citizenship or leadership report forms,
additional Tennessee
4-H Report Forms for the past three years (2001, 2002, 2003)
may be included for one project other than leadership or citizenship.
A form may also be included for work done in calendar year 2004. |

Seniors may include project manuals and other supporting records
for leadership and citizenship plus one other project for the
past three years (2001, 2002, 2003) plus the current year (2004). |

Junior high members should include project records for 2003
and for any work done in 2004.

| Leadership and citizenship record scorecards are
slightly different than the standard record scorecard for other
projects. Copies are available from the state 4-H office. |
