Forms

The following are available to managers of 4-H Foundation accounts. The forms may be completed onscreen, printed and faxed or mailed to the Tennessee 4-H Foundation office at:

Original copies of receipts are required when requesting personal reimbursement.

Extension staff may not receive travel reimbursement from the Foundation, however the county, region or department may invoice the Foundation for reimbursement of travel expenses when appropriate.
